Program Overview:
Mission: To provide a spectrum of primary and secondary prevention, early intervention and wellness focused activities to prevent diabetes, as well as to improve diabetes self management care.
Vision: Preventing the early onset of diabetes and supporting individuals and families through education, creative physical activity and exercise programs. Provide emotional and spiritual support that will ultimately allow them to re-embrace Laguna traditions that promote health and wellness of the mind body and spirit.
The Fitness Center is open to tribal members, and Native Americans age 16 and over, their spouse and anyone employed under the POL tribal government free of charge. Any other individuals are charged $15/3 months or $30/1 year.
Programs/Events:
Diabetes Support Group meets weekly on Tuesdays beginning at 1:00pm at the K-Center or Fitness Center. The clinic is supported by key staff from the local hospitals and programs. The Diabetes Care Coordinator facilitates the support group meetings, provides education, support & information, as well as provides home visits to individuals with Diabetes. The group offers a safe, supportive and informative environment for people. Special population activities are coordinated and held at the fitness center and other community sites. Personal Trainer assistance is available upon request.PAK Activity programs implemented in local schools weekly to promote physical activity. Health Screenings and Special Events (Go Red Event and Diabetes Conference).
News/Announcements:
All participants are required to fill out enrollment, health history and assessment forms. Vitals required for enrollment are height, weight, blood pressure, blood glucose, oxygen saturation, heart rate. Body measurements are available.
Diabetes Wellness Groups meetings: 1pm to 3pm, every Tuesday. Groups are open to people living with Diabetes or newly diagnosed. Clinical support provided by the I.H.S. Diabetes Educators & Dietician.
Field use: For use of the new Baseball and softball fields, complete the User form and submit for review to the Program. Tribal members and other Native American teams utilize the fields for free. Charges apply to others. Use based on availability and scheduling.
