Community Health and Wellness

Provides direction, oversight, support, guidance to all Department level programs and ensures quality, responsive services are provided in a manner that meets the needs of the community and complies with tribal laws, regulations, standards, Ethics and customer satisfaction. The Departmental offices are located in the Old Laguna village, Laguna Pueblo in the Community Services complex.

Department on Call:  Behavioral Health programs maintain 24/7 on call status for crisis assistance. CHR  maintains 24/7 non-Emergency patient care assistance.  Benefits Office maintains on call status for assistance to Veterans and their families.  All calls are directed to the Laguna Police Department.  Department staff provides support for emergency management and public health emergency incidents. All crisis calls are handled through Laguna Public Safety Department Dispatch- 552-6685 and referred appropriately.

Community Based Outreach, Education and Support Programs Services

Health, Wellness and Medical Programs

CHR Programs offer services focused on health promotion, disease prevention, wellness, health recovery group support and health screenings for chronic disease in the home and community. Programs for elders, children, adults, prenatal clients, men, and women are available. Provides CPR and First Aide Community Classes.

Fitness Center funded through the Special Diabetes Grant, and provides a state of the art fitness center equipped with exercise equipment, staff for personal trainer assistance, diabetes education and information provided. Provides bi-weekly Support Groups for community every  Tuesday at the from 1pm to 3pm at the K Center or Fitness Center. Nutrition counseling available by staff Registered Dietician. Transportation coordinated to attend meetings. Hours of operation- Monday to Wednesday 6am to 8pm; Tuesday and Thursday 6am to 8am- 8am to 10:00am (special population programming), 10am to 8pm. Friday 6am to 6pm.

Sports & Wellness located at the Kawaika Center funded by the Special Diabetes grant, provides open gym, physical fitness and sports programs targeting physical activity, inclusion and diabetes prevention for age groups 7 to 65 years of age. Coordinates fun runs, special public events, and supports school based and villages based programs for wellness.

Laguna Cultural Enrichment Program located at the Kcenter offers ongoing, center based, and special village based cultural and traditional instruction in classes, and experiential learning in arts, crafts, cooking, language, and other aspects of culture. Classes the program is organizationally part of the Sports and Wellness program.

K’awaika Center  formerly old LA High school is a revitalized building in Laguna, designated as a community center for person’s age 2 years to age 100+. The center is a multi-functional location that offers physical wellness programs, training and education, cultural classes, exercise programs, meeting space, gallery, computer room and movie  a Theater. Public space is available for private functions through individual contracts.

Behavioral Health Services

Laguna Service Center offers counseling, therapy and support for persons who want to make a life change. Services are provided for adults and children. Services include intake and assessment, individual and group counseling and MRT, and therapeutic support groups. Operates a certified DWI school. Provides school and community based prevention programs and activities for children, and their families. Provides 24/7 crisis intervention. BHS staff is professionally licensed and have specialty areas of expertise including substance use and abuse, mental health and personal life crisis, marriage and family counseling, gambling and other addictive behaviors.  Prevention programs for suicide prevention, prevention of Under aged drinking, Gambling addictions, and experiential youth outings are provided.   Provides DV  24/ 7, crisis   shelter housing   for  women and their children advocacy, and support. Coordinates court-ordered visitations for tribal members. Provides access to counseling and facilitates support groups for victims and families, and perpetrators. Provides parenting classes, coordinates training and education and awareness sessions at schools and at public events.

Social Services Program Provides protection, support, advocacy for adults and children who are members of the Pueblo of Laguna. Implements ICWA standards to ensure child protection. Provides general assistance, case management in accordance with Court Orders and care plans. Coordinates Foster care and adoption for children. Transportation Services: 

Shaa’ srka Transit:    Provides demand responsive fixed   route,  route  deviation within the local area. Services are from 6:30am to 5:30pm, Monday thru Friday. Fees for services range from .50 to .75 for children, elders, adults and persons with disabilities.

Laguna Medical Transit: Offers safe, reliable rides to qualified persons in need of weekly, local dialysis services.  The programs is coordinated through the Shaa’srka program. Services are provided six days per week for dialysis. And limited rides to local medical appointments.

Benefits and Support Programs

Benefits Services  Program :  Provides assistance with enrollment and advocacy for benefits  to community and tribal members for Medicaid, Medicare, Disability, SSI, I.H.S. and providers referrals to other benefits programs.  Assists with TANF, ISD office connections, PEMOSSA enrollment. Offers specific services to Veterans and their families to ensure access to benefits and quality health care services, benefits, and compensation is available. 

 
Public Health  Nursing Program:  RN  field   based   education and case  management  injections, wound care, health education, self-care training, referrals, advocacy and support, school based health support, assistance with public health and public health emergency management.  Providers care and services with Medical staff at all medical facilities for field based care.
 
Laguna Public Health Education Program:   The program is designed to support individual, family and community with needs for health education, information, self-care, motivational coaching and wellness practices in a culturally relevant manner. Chronic disease prevention, and management, health promotion and disease prevention education and support can be provided on an individual or group level. Implements evidence based and promising practice based curriculum. Implements key health education programs focused on CVD, Tobacco, Exercise, Nutrition, and support.
